1) Please fill out all fields on this form. 2) After the form is submitted, you will be asked to print one copy of the Rental Registration form AND print one copy of the Rental CO application AND print one copy of the Smoke/CO application. 3) Make the required payment. You have three options to make the payment: MAIL -OR- DELIVER THE APPLICATION IN PERSON WITH THE FEE TO: BOROUGH OF WEST LONG BRANCH 965 BROADWAY WEST LONG BRANCH, NJ 07764 -OR- PAY THE FEE ONLINE. In order to submit the application for a Rental CO, the following conditions MUST be met: Required Fields: 1) Scheduled Tenant Change Date 2) Parcel Data 3) Street Address 4) # of Rental Units 5) Owner Name 6) Email 7) Telephone (Day) Conditions To Be Met: 1) YOU MUST LIST EVERY TENANT THAT WILL BE ASSOCIATED WITH THE RENTAL a) This can be done by clicking "Add Tenant Information" b) You must enter a FIRST NAME and LAST NAME for EVERY TENANT 2) You must check the checkbox next to the text "I ACKNOWLEDGE RECEIPT OF THE BOROUGH OF WEST LONG BRANCH CODE ENFORCEMENT ORDINANCE" 3) You must click the button PRINT ORDINANCE SHEET 4) Street Address under "Owner Information" and "Property Information" CANNOT BE THE SAME. If they are, Unit # is required 5) You cannot have more than 2 Rental CO's per year
