the owner shall obtain a Smoke detector Carbon Monoxide alarm certificate
or be subject to a $500.00 Fine as per N.J.A.C. 5:70-2.12(b)

Fees

$45.00 - Received more than 10 business days prior to the change of occupancy. $90.00 - Received 4 to 10 business days prior to the change of occupancy. $161.00 - Received fewer than 4 business days prior to the change of occupancy.

REQUIRED, CHECK EACH BOX:

Devices less than 10 years old	I confirm that all smoke detectors on the premises are less than ten years old and, where applicable, are of the ten-year sealed battery type.
Underwriter standards met	I confirm that all smoke detectors installed meet the Underwriters Laboratories (UL) _ standards
On each level dwelling	On each level of the dwelling, including basements: excluding attic or crawl spaces: and
Outside each sleep	Outside each separate sleeping area: and
CMD installed	Carbon Monoxide Detector installed in the vicinity of each sleeping area. (N.J.A.C 5.70-4.19(D))
CMDs work	All Smoke and Carbon Monoxide Detectors are in working order.
Fire Extinguisher required	Min 2A:10BC Fire Extinguisher required to be mounted within 10 feet of the kitchen
